Thank you for your interest in the CNA/HHA Program at Contra Costa College. Following are detailed instructions explaining the steps required to be accepted into, and register for the CNA/HHA Program. Please read them carefully.

Complete and submit a College Admissions Application if you are a new student at Contra Costa College. Applications are available on line, or from the Admissions and Records Office, SSC 115. |

Obtain CNA/HHA Admissions Application packet from the Nursing Office, HS, 103. |

Seek Financial Assistance if qualified, EOPS office, SSC 102. |

Complete all pre-requisites for the CNA/HHA program and apply to the program prior to enrolling in Nurs 202-CNA/HHA. |

College Assessment tests – Assessment of new students is required in the areas of reading, writing, and math. Assessment tests are available on a drop-in basis at the Assessment Center, SSC-104. You will need your student ID #. See CNA/HHA Admission Requirements. |

CNA/HHA Admissions Application -- This form must be submitted to the Nursing Office, (HS 103). |

| |
|
LiveScan form and State form HS283B, Nurse Assistant And/Or Home Health Aide Initial Application -- This form will be submitted to the Nursing Office (HS 103) for transmittal to the Dept. of Health Services. ** See below regarding LiveScan fingerprinting. |

|
LiveScan fingerprinting -- Go to cashier’s office and submit fee for LiveScan fingerprinting. Have fingerprints taken at Campus Police Services, R Building.

IMPORTANT: Any student who has a declared conviction on the Department of Health Services HS 283B application will not be allowed to enroll in Nursing 202 unless he/she submits a letter of approval from the Aide and Technician Certification Section in Sacramento. (SEE BOX BELOW REGARDING BACKGROUND INVESTIGATION). |

The nursing office will notify you of your acceptance to the CNA/HHA program. |

Upon acceptance into the CNA/HHA program, enroll in Nursing 202 – Certified Nursing Assistant – Home Health Aide, 12 units. |

Class meets Tuesday through Friday |
| |
The first four weeks class will meet on campus. |
| |

Tuesday , Wednesday, Thursday and Fridays |
| |
Beginning the 5th week through the end of the semester: |

|
Tuesdays and Wednesdays classes will meet on campus 8:00AM-2:00PM |
| |
|
Thursday and Fridays will meet at the clinical site from7:00AM – 1:00PM |

Enroll in English 103 -- 0.5 units -- 18 hours throughout the semester by your own arrangement. Signing up for this class allows you to access tutors and other academic help. Your instructor will place assignments for you at the Student skills lab in the library. |

Enroll in Medic 109 – CPR for the Healthcare Provider – 0.2 units. Be sure that the section in which you enroll will be complete by week 4 of the semester. OR complete CPR for the Healthcare Provider through a private agency |

Enroll in Nursing 204 – Orientation to Nursing Assistant – 1 unit. |

Complete and submit a Health Form to the Nursing Office (HS 103). This form is enclosed. The required Health Form includes: A complete Physical Exam, proof of immunization(s) listed on health form, and Negative TB test. **
Your medical provider must fill out the form included in your acceptance letter. |

ESL Students: Help with English as a Second Language can be found at our ESL Dept., ext. 4416 or in room AA 109. You will have a greater chance for success if you test at an English level of 3 or greater. Our textbook is written at a 9th grade level. |

DSP&S: If you have had difficulty in school in reading, math, language and perception it is highly recommended you sign up for free testing as soon as possible in DSP&S (Disabled Students Programs & Services), ext. 7220 or in room SSC109. |
